This module explores the various ways that patients’ needs, in terms of population health, can be boosted through the right use of technology. It begins by looking at the benefits of correct—and careful—use of electronic health records. It then explores how good software can help consolidate information (from claims to revenue and patient records) and offer data analysis about patient outcomes on a population level. Last, the module looks at the advances in health information technology (HIT)—from appointment-making technology to blood pressure cuffs with telemonitoring capabilities—that can help patients and healthcare providers stay connected while patients manage a chronic condition.

When we can connect effectively with patients when they are away from a hospital or doctors’ offices, we can help them better manage their care and avoid unnecessary doctor visits or hospitalizations.
