Description: Even though chronic obstructive pulmonary disease (COPD) is the fourth leading cause of death worldwide, it is strikingly under-diagnosed, undertreated and misunderstood. This course describes the prevalence and pathophysiology of COPD, the proper methods of assessment and management, the most effective pharmaceutical and non-pharmaceutical treatments and interventions, and the case manager’s role in helping patients manage COPD.

Business goals: When the CM team completes the COPD course, the hospital can reasonably forecast:
» Improving COPD patient outcomes by 40 percent
» Reducing readmissions among COPD patients by 40 percent
